Guide

How to claim a medical reimbursement from NAS

To claim a medical reimbursement from NAS, fill in the NAS Reimbursement Claim Form, have your doctor complete, sign and stamp the medical section, then email the form with your invoices, receipts and reports to [email protected]. This free tool fills in the form in your browser and gives you a ready-to-print PDF.

  1. Enter the member details

Add the patient's name and card number as printed on the NAS card, plus date of birth, email and mobile number. NAS asks for BLOCK letters, so the PDF prints your answers in capitals.

  2. Add your invoices and bank details

List each invoice you are claiming (up to six fit on one form); the total per currency is added for you. Then enter your bank details. For transfers within the UAE the bank name, account holder and IBAN are mandatory; for transfers abroad, complete every field.

  3. Download the PDF and print it

Open the filled PDF and print it. The boxes you still have to complete by hand are highlighted in yellow: the physician's signature, stamp and date, and your own signature and date.

  4. Have your doctor complete the medical section

The treating doctor completes section 2, then signs and stamps it. You can also enter the medical details in this tool first to save them time.

  5. Send the claim to NAS

Email the signed form and your supporting documents to [email protected], or send them by courier, which NAS prefers. If you submit online, keep the original documents, because NAS may ask for them to finalize your claim.

Documents to send with your NAS claim

  • The completed NAS Reimbursement Claim Form (mandatory)
  • The medical section, fully completed, with the treating doctor's signature and stamp
  • Itemized invoices and receipts showing you paid by cash or credit card
  • Prescriptions, if you paid for medication
  • Radiology, imaging or lab reports, where applicable
  • A discharge summary and medical report, for inpatient claims
  • A copy of the NAS approval email, if you received one
  • A translation into English or Arabic, for any document in another language

Frequently asked questions

Where do I send my NAS reimbursement claim?

Email it to [email protected]. NAS also accepts claims by courier and says courier is preferred, because some insurers can only reimburse against original documents. NAS sends an automatic acknowledgement when it receives your email.

Which NAS form do I use for medical treatment?

Use the Medical Reimbursement Claim Form. It covers all treatments except dental, which has its own Dental Reimbursement Claim Form.

Who fills in the medical section of the NAS form?

Your treating doctor. The section must be fully completed in BLOCK letters and carry the doctor's signature and official stamp.

Do I need NAS approval before treatment?

NAS prior approval is required for all non-emergency hospitalizations. Before admission, email a detailed medical report and cost estimate on the doctor's signed and stamped letterhead to [email protected]. NAS says an approval is valid for 14 days.

How does NAS pay a reimbursement?

By wire transfer or cheque, using the details on your claim form. NAS does not pay in cash. NAS accepts no liability for incorrect bank details, and transfers outside the UAE may attract bank charges.

What language do my documents need to be in?

English or Arabic. Documents in any other language must be translated before you submit them.

How do I check the status of my NAS claim?

Call the NAS 24/7 helpline on 800-2311 or email [email protected] if your settlement has not arrived.

Is this the official NAS form?

This tool is unofficial and not affiliated with NAS. It fills in NAS's own claim form, so the PDF you get is the official layout. Check the details with NAS before you submit, because requirements can change.

Does this tool send my details anywhere?

No. The form is filled in inside your browser and nothing you type is uploaded. If you tick "Save my details", your member and bank details are kept in this browser only, and you can delete them by unticking the box.
