Fill in what you have and download the MetLife claim form as a ready-to-print PDF. Every field is optional, and nothing you type leaves your browser.
Guide
MetLife prefers claims through the myMetLife website or app (log in, open cash claim, enter the details and submit). If you cannot use myMetLife, fill in the Medical Claim Reimbursement Form, have your doctor complete the physician section, and send it with your invoices and medical report within 30 days of treatment, and never later than 90 days. This free tool fills in the form in your browser.
Add the employee's (policy holder's) and the patient's full name, date of birth and nationality, plus the policy and certificate numbers printed on your MetLife medical card. The same person can be both. Every field is optional: you can open the PDF at any point and finish the rest by hand. The PDF prints your entries in capital letters, as the form asks.
Choose wire transfer or cheque and enter the total amount you are claiming and its currency. The form has no bank boxes: MetLife pays to the bank details saved in your myMetLife profile, so update them there first.
Page 2 is for the attending physician: chief complaint, diagnosis, when the symptoms started, treatment, and the physician's name, address and phone number. Enter what you know, or leave it empty for the doctor to write by hand.
Open the filled PDF and print it. The areas still to complete by hand are highlighted in yellow: your signature and date on page 1, and the physician's signature and stamp and the document checklist on page 2.
If you are covered through your employer, use the myMetLife website or app or ask your HR team. For an individual policy, MetLife lists [email protected] for copies and PO Box 371916, Dubai for originals. Keep the original documents: MetLife may ask for them later.
Through myMetLife (website or mobile app): log in, go to cash claim, enter the details and submit. If you cannot access myMetLife, send the completed claim form and documents instead. MetLife lists [email protected] for individual claims, with originals to MetLife Claims Department, PO Box 371916, Dubai. Employees covered by a company policy can also ask their HR team about the submission process.
The form asks for all claim documents within 30 days of the incurred date, and says claims submitted more than 90 days after it may be denied. MetLife’s claims page gives 90 days as the limit for medical claims. Check your policy terms for the exact period.
The treating doctor. The physician section on page 2 needs the chief complaint, diagnosis and how long the patient has had the condition, and carries the physician's signature and stamp. You can enter the details here first to save the doctor time.
Not on this form. MetLife says the bank detail must be updated on myMetLife, and pays wire transfer reimbursements to that account. The form offers wire transfer or cheque.
