What a reimbursement claim covers
A reimbursement (or “pay and claim”) is for treatment you paid for yourself instead of using your insurance card: a clinic outside the network, an emergency abroad, a pharmacy that did not accept direct billing, or a provider that asked for payment up front. The insurer checks the claim against your policy and pays the eligible amount to your bank account.
Every insurer uses its own claim form, but they ask for the same things: your member and policy details, the treating doctor’s diagnosis, signature and stamp, an itemised list of invoices with amounts and dates, and the bank account to pay into. The forms here print those details in the right boxes so you only add the signatures by hand.
Sukoon, NAS or MetLife: which form do you need?
Check the name on your insurance card. Sukoon Insurance (formerly Oman Insurance) is the insurer: if your card says Sukoon, use the Sukoon reimbursement form and submit it through the mySukoon app, the Sukoon medical portal, a branch or your broker.
NAS Administration Services is a third-party administrator that handles claims on behalf of several insurers. If your card carries the NAS logo or your policy documents name NAS as the claims administrator, use the NAS reimbursement form and submit it to NAS, even though the policy itself is issued by another insurer.
MetLife UAE issues its own medical plans. If your card says MetLife, use the MetLife reimbursement form, which is for in-patient treatment, and submit it through myMetLife or MetLife Gulf customer services. Bank details are not on the form: MetLife pays to the account saved in myMetLife.
If you are unsure, the member services number on the back of your card confirms where claims go. Submitting the wrong insurer’s form delays the payment.

Daman, Cigna, AXA (now GIG Gulf), Orient, Nextcare and other UAE insurers and administrators each publish their own PDF form on their website or member app. The process is the same as the guides here describe: fill in the form, have the doctor complete the medical section, attach original invoices and reports, and submit within the deadline your policy states, typically 90 to 120 days after treatment.
New forms are added when enough people ask for them. The guides on the Sukoon, NAS and MetLife pages still answer most general questions about documents, deadlines and bank details.
Frequently asked questions
How long do I have to submit a medical reimbursement claim in the UAE?
Most UAE policies allow 90 to 120 days from the date of treatment. Sukoon accepts claims up to 120 days after treatment; NAS and other administrators state their own deadline in the policy terms. Submit as soon as you have the invoices and reports, because a late claim is normally rejected.
Which documents do I attach to a reimbursement claim form?
The completed and signed claim form with the doctor’s section filled in, stamped and signed; original itemised invoices and payment receipts; the medical report or prescription; lab or radiology results if claimed; a copy of your insurance card and Emirates ID; and your bank details or IBAN certificate for the refund.
Can I fill in the claim form on my phone?
Yes. The forms here work in any modern browser on a phone, tablet or computer, and nothing you type is uploaded. You download a finished PDF, print it, add the signatures and stamp, and submit it through the insurer’s app, portal, branch or courier as the guide describes.
How long does a UAE insurer take to pay a reimbursement?
Insurers usually settle complete claims within two to four weeks of receiving them. Sukoon aims for three weeks on eligible claims. Missing documents or an unsigned doctor’s section are the most common reasons for delay, so check the form before sending it.
Unofficial helpers, not affiliated with or endorsed by any insurer or authority. Requirements change: check them with the provider before relying on them.
